Step 1:
Pick 5 numbers

For each play choose five (5) numbers from 1 to 58 in the left part of the play area OR select the Quick Pick (QP) circle in the lower right of the play area if you want the terminal to choose your numbers. You can also choose less than five numbers and have the terminal complete your play slip by selecting
Quick Pick (QP).

Use only blue or black ink, or pencil. If a box is marked in error, choose “VOID” for that panel and move on to the next panel. DO NOT ERASE.

Select one (1) number from 1 to 5 in the right play area OR select the Quick Pick (QP) circle in the lower part of the play area if you want the terminal to choose your numbers.

Number of Draws

For your convenience, tickets can be purchased for up to fourteen (14) consecutive drawings, including the current drawing, by selecting the desired number in the NUMBER OF DRAWS area atop of the first play area.

For example, if you want to play in the next four consecutive drawings, fill in the “4” circle in the NUMBER OF DRAWS area.

Remember, the number of draws you select applies to each panel.

NOTE: Prize payments for the top two prizes last for a lifetime. There is also a minimum period of 20 years; if a winner dies before this time the rest of the money will go to the winner’s estate. Overall odds of winning a prize are approximately 1:8.47. A player may win in only one (1) Prize Level per single play and shall be entitled to only the highest Prize Level for that winning play. Under certain conditions, the Grand Prize and the Second Level Prize may become pari-mutuel.

In accordance with the Mississippi Lottery Corporation prize payment policy, all instant game prizes must be claimed within 90 calendar days of the announced end of the game. Postmarks shall not constitute satisfaction of the 90-day requirement, and the risk of mailing tickets remains solely with the player.
In accordance with the Mississippi Lottery Corporation prize payment policy, all draw-style game prizes must be claimed within 180 calendar days after the draw date. Only original tickets will be accepted to claim any prize. Copies, photographs, or altered tickets are not valid.
Prizes under $600 may be claimed at any Mississippi Lottery Corporation retailer. Prizes between $600 and $99,999 must be claimed either at the Mississippi Lottery Headquarters or by mail. A completed Mississippi Lottery Winner Claim form, proper identification (ID), and the original ticket are required for all prize claims of $600 or more. Acceptable identification must verify name, signature, photo, age and Social Security number. If claiming by mail, send all required documentation to the Mississippi Lottery Corporation, P.O. Box 321462, Flowood, MS, 39232. The Mississippi Lottery is not responsible for lost, delayed, or misdirected mail.
Prizes of $100,000 or more must be claimed in person at the Mississippi Lottery Headquarters located at 1080 River Oaks Drive, Bldg. B-100, Flowood, MS, 39232.
For instant scratch-off games, the number of prizes is established at the time of printing and will decrease as prizes are won.
All tickets, transactions, and players are subject to the Mississippi Lottery Act, Mississippi Lottery rules, and regulations, and applicable laws. Millionaire for Life™ is a new national draw game from the Multi-State Lottery Association (MUSL), offering players the chance to win the incredible grand prize of a $1,000,000 a year for life!

Are the top prizes paid as annuities?
Yes. The Grand Prize and Match 5 prize are annuity prizes.
Annuity prizes include a 20-year guaranteed payout.
Will I really win $1 Million for Life?
Yes, if the winner chooses the annuity option. Millionaire for Life rules define Measuring Life as the natural life of the individual who the Lottery determines to be a valid Prizewinner.However, for the following Claimants, the Measuring Life shall be fixed at twenty (20) years:
• A legal Entity other than an individual
• More than one natural person who claims a Prize on the same Ticket.
